Medical Subsidy for Children

| Eligibility |
| Children who are enrolled in the National Health Insurance or Social Insurance living in Toyota city, until 15 years old (until the first March 31st after completing 15 years old) are eligible to apply. |
| Coverage of the subsidy |

Consultations made in Aichi Prefecture: The portion of the medical expenses (30%) that should be paid by the patient are covered by this medical subsidy. As a result, children consultations will be free of charge until before they complete 15 years old (until the first March 31st after completing 15 years old). When consulting a physician, please do not forget to present the “Medical Subsidy for Children“ card and the Health Insurance card at the counter. |
| It is necessary to notify the City Hall in the following cases |
| 1. When a newborn baby is qualified for application 2. Change of address to Toyota City (or when coming from another country) 3. Change of address to another city or country 4. Change of the child’s name or change in the Health Insurance card |

| Applications should be made at the Toyota City Hall, “Living and Welfare Division”, East Building, 1st floor or at the “Citizens’ Division”, South Building, 1st floor. |
| Applications for Reimbursement Necessary Documents |
| 1. Receipt of the medical expenses or a certificate from the medical institution (a sample of the certificate can be obtained at the “Living and Welfare Division”, the “Citizens’ Division” or sub-branches of the City Hall). 2.
