Renewal of Certificate of Limited Health Cost and Certificate of Reduced Standard Cost-sharing
It’s time of the year to renew (new application) your “Certificate of Limited Health Cost/Certificate of Reduced Standard Cost-Sharing”. A person under 70 years old who is enrolled in the National Health Insurance, can have medical fee (incurred during hospitalization) reduced. Target people: Person enrolled in the National Health Insurance whose household is exempt from the residential tax (eligible for Certificate of Limited Health Cost/Certificate of Reduced Standard Cost-Sharing) and who is under 70 years old (eligible for Certificate of Limited Health Cost). Necessary documents: Renewal application form, health insurance card, head of household’s personal seal (inkan), ID of the person who comes to do the procedure such as driver’s license or alien registration card, letter of attorney (if a person from different household does the procedure), the Certificate that expired in the end of July 2011 (if you are renewing). Period: From August 1st (Mon) to 31st (Wed) (if you need a certificate that can be used from August 1st). Where to apply: Toyota City Hall - Health Insurance and Pension Division, or at Asahi, Asuke, Inabu, Obara, Shimoyama and Fujioka branch offices (renewal application by mail is also accepted, but it might take about 1 week for you to obtain new certificate). Other information: You may not be able to receive the certificate if you have unpaid insurance premiums. For further information, contact Health Insurance and Pension Division. Submission of current status report of single parent allowance
People who were receiving allowance for single parent (from Toyota City, Aichi Prefecture & National Government) as of July 31, 2011 must submit Current Status Report so that they can continue receiving the benefit. Period for submission: From August 1 (Mon) to 31 (Wed), from 9 am to 4 pm, except on Saturdays, Sundays and National Holidays. If the documents are not submitted on time, the payment of the allowance in December will not be made. Paperwork required and place for submission: The information was sent in the end of July to those currently receiving the allowance. Other information: In case of change of address, name or other status, please inform about it on the day you submit the paperwork. Place: Industry and Culture Center Content: Bazaar, interchange playing a mini game with people from different countries and other attractions you can experience, such as bamboo flute making, face painting, world cosplay. Other information: Admission free. All proceeds from sale of the day, such as from bazaars, are donated for the reconstruction assistance to the areas stricken by the Great East Japan Earthquake.
